America’s Health Insurance Plans
America’s Health Insurance Plans (AHIP), www.ahip.org, is the national trade association representing approximately 1,300 health insurance plans that provide coverage to more than 200 million Americans. Health insurance plans have a strong track record of encouraging prevention and evidence-based care for individuals with chronic conditions, and quality of care reporting such as HEDIS® measurements demonstrate that health plans have improved rates of comprehensive diabetes care for their members. AHIP and its member health plans are addressing the challenge of prevention and early detection of diabetes in the Medicare population through targeted outreach to members and communities; creating incentives for members to get screened for diabetes and related conditions, including body mass index and hypertension; creating and funding community screening and early intervention programs; and developing programs emphasizing culturally and linguistically appropriate services and resources to assist members in navigating their health care and health benefits.

American Association of Diabetes Educators
The American Association of Diabetes Educators (AADE) represents the healthcare professionals who teach people with diabetes, or those at risk of diabetes, how to control the disease through Diabetes Self Management Training and Education (DSMT). Diabetes educators are healthcare professionals who focus on helping people with, and at risk for, diabetes and related conditions achieve behavior change goals which, in turn, lead to better clinical outcomes and improved health status. Diabetes educators apply in-depth knowledge and skills in the biological and social sciences, communication, counseling, and education to provide self-management education/self-management training.

Diabetes Education is an effective means of controlling diabetes. While the cost of a diabetes education program is less than the average cost of an emergency room visit for an acute diabetes-related complication, many individuals do not even realize they have diabetes until confronted with an acute health episode. Diabetes screening is an essential component of the Medicare program to help identify individuals with diabetes, or those at high risk, so that they may begin to take the right steps to learn how to manage their disease through a DSMT program.

The American Diabetes Association (ADA) is the national's leading 501(C)3 nonprofit health organization providing diabetes research, information and advocacy. Founded in 1940, the ADA conducts programs in all 50 states and the District of Columbia, reaching hundreds of communities. The ADA funds research, publishes scientific findings, provides information and other services to people with diabetes, their families, health professionals and the public, and is actively involved in advocating for scientific research and for the rights of people with diabets. Educating older adults about diabetes and hypertension screening and Medicare Part B Preventive Benefits is important to the National Council on Aging (NCOA).  NCOA, a leader in promoting healthy aging nationwide, has initiated a social marketing campaign, "Connect the Dots ~ Diabetes and Your Heart: How Medicare Helps" targeted to older adults to break down myths and increase knowledge of the incidence of both diabetes and hypertension; to build increased awareness of Medicare Part B Preventive Benefits that help pay for screening, management and treatment of diabetes and hypertension, and to track whether these educational efforts have a positive impact on the health outcomes of attendees to Connect the Dots events at senior centers across the U.S.  Information about NCOA's campaign is available at www.ncoa.org/connectthedots.

Care Improvement Plus supports the Medicare Diabetes Screening Project in order to: 1) raise awareness of the issue of diabetes among Medicare beneficiaries; 2) encourage evidence-based medicine practices, including appropriate preventative care and medication compliance; and, 3) to help beneficiaries better understand their medical conditions, such that they may make informed choices regarding their Medicare coverage.  Care Improvement Plus is a unique Medicare Advantage Part D (MA-PD) health plan exclusively designed to meet the needs of Medicare beneficiaries living with diabetes, heart failure, chronic obstructive pulmonary disease (COPD), and/or end stage renal disease (ESRD). Care Improvement Plus offers more benefits than Original Medicare and many other Medicare Advantage plans, including care management programs and in-home health monitoring.
